•  An application for enrollment and a current medical evaluation must be obtained before attending adult day services
•  The applicant or their primary care giver will be required to attend an interview iwth the center director.
• Each applicant accepted into the adult day care will have a written plan of care that is givers final approval.  This plan will be reviewed every 3 months of individual continued enrollment.
